To aligned with the FA, who are rightly concerned that written parental consent is given for every child participating in grassroots football, we have a new Club Membership & Consent Form for this season (and beyond). This also captures vital information regarding your child's health and wellbeing requirements which are then logged by the club and relevant coaching team.
This form is mandatory and every player must have one completed and returned to the club.
